PATIENTS

Before you come in

New patients should print the packet and complete it before the first visit. Current patients can call about refills, results, and follow-up.

Insurance

We accept most major plans, including Medicare, Medicaid, and commercial insurance. If you are not sure your card is covered, call 330-799-8752 before you schedule and we will check.

We do not handle workers’ compensation claims, and we do not bill auto insurance.

New patient forms

Print the new patient packet, complete every page, and bring it with you. A photo ID and your insurance cards should come too.

  • A photo ID
  • Your insurance card
  • A list of medicines, or the bottles themselves
  • Names of other doctors you see
  • These forms, or an extra 15 minutes if you would rather fill them here

Refills and results

Call the office during office hours for a prescription refill or to ask about a result.
